en A novel technique of bladder neck reconstruction during open suprapubic prostatectomy in patients with benign prostatic hyperplasia Clinical Medicine Clinical Medicine مقاله پژوهشی Original Research Article Background: Postoperative hemorrhage represents a significant complication following open prostatectomy. This study aimed to compare the clinical outcomes of conventional open prostatectomy with a modified technique incorporating additional sutures for bladder neck reconstruction. Methods:  This retrospective cross-sectional study analyzed the medical records of patients diagnosed with benign prostatic hyperplasia (BPH) who underwent open prostatectomy at Imam Khomeini Hospital, Urmia, Iran, between 2018 and 2022. Patients were categorized into two groups based on the surgical approach: conventional open prostatectomy and the modified technique. Demographic data, preoperative and postoperative hemoglobin levels, blood transfusion requirements, length of hospital stay, and preoperative prostate-specific antigen (PSA) levels were compared between the groups. Additionally, mean prostate volume, packed cell volume, and the presence of underlying comorbidities were assessed. Results: A total of 208 patients underwent open prostatectomy: 108 received the modified technique, and 100 underwent the conventional approach. The mean age of the study population was 69.7 ± 4.10 years. No significant intergroup differences were observed in baseline characteristics, including age (p=0.131), body mass index (p=0.076), prostate volume (p=0.643), and prostate-specific antigen (PSA) level (p=0.812). Patients in the modified prostatectomy group exhibited a significantly shorter length of hospital stay (5.4 ± 0.9 days) compared to the conventional group (p<0.001). The requirement for packed red blood cell transfusions was significantly lower in the modified prostatectomy group (10.2%) compared to the conventional group (29%) (p=0.002).
