Mucormycosis is an aggressive fungal infection that can lead to fatal complications in patients with underlying compromising conditions if not promptly and vigorously managed.
A 65-year-old female from Ardabil city, Iran, with Type 2 diabetes mellitus (DM) presented with left-sided hemiplegia and hemiparesis. Within five days of admission, she developed pain, left-sided facial swelling, fever, and chills. She has also been undergoing treatment for brucellosis for the past two months. Physical examination showed a necrotic lesion on the hard palate and upper lip. Laboratory tests were conducted, and subsequent nasal endoscopy revealed the presence of mucormycosis hyphae. A sinus MRI indicated invasive fungal sinusitis and mastoiditis. Palate lesion excision pathology confirmed mucormycosis. The patient received amphotericin B and posaconazole, with surgical debridements and additional treatments for comorbidities. Surgical interventions included maxillary sinus wall removal and nasal cavity exploration. After completing a six-month course of maintenance drugs, the patient made a full recovery.
